- Name
- Profession/qualifications
- Professional registration number, membership or affiliation
- Type of pet/s used
- Who the pet is registered with and the date of registration
- Details of services offered
- Contact details
- Declaration of professionalism (copy the following into your email) – "I declare that the information I have provided is true and correct to the best of my knowledge."
- Photo if desired (not exceeding 1Mb in file size)
